  If you are looking for Baptismal,  First Holy Communion, Confirmation or Marriage records, your church of Baptism is the first point of contact for finding your sacramental records.

  
  

  If you were baptized at our parish and have a record request to make of St. John the Apostle Church, be sure to have the following information available to help us retrieve your records:
   
   Your Full Name (including maiden if married)
   Date/City/State of Birth
   Name of Parents (Please include Mother’s Maiden Name)
   Date or Approximate Date of Sacrament
   Name of Church and email address where official document can be sent
   Your contact phone/email
